"Suggest an app" popup window should have more fields for user input (+ administrators can decide)

The "Suggest an App" functionality (popup window) has only 3 fields (Title, URL, Description) for user input. It would be great if more fields (all available fields for Apps) can be displayed (E.g. Contact, Icon, Categories, etc.).
Best case: Administrators can add/remove the additional fields (+ decide if fields should be required) for the "Suggest an App" popup window that is displayed to the users.

Great idea! It is also very important for us to be able to implement an individual description how to suggest an app and what additional information might be needed.
